On this page, you will find the forms required by Social Housing Investment Funds investors to open an account, deposit, transfer, withdraw funds, and authorize certain other activities. Opening a New Investment Account


A Guide to Completing Your Account Application Forms

We want to make the account-opening process as straight-forward as possible for you. Ensuring that the investment account forms are complete and accurate will help to prevent delays in establishing your account with the Social Housing Investment Program.

A Guide to Completing Your Investment Account Forms is designed to take the guess-work out of the account opening process.

Investment Account Application * form must be mailed

Use this form to open a new Social Housing Investment Fund account. Be sure to include all relevant attachments, which are listed on the first page of the application.

Defining your Investor Profile & Account Objectives and selecting investment funds

This article is intended to guide you through defining your corporation’s Investor Profile and Account Objectives and provide direction on selecting your corporation’s Social Housing Investment Funds.

Resolution of Directors – New Unitholder * form must be mailed

This form records the officers of the corporation to be given authority for the Social Housing Investment Fund accounts. Must be accompanied by a completed Federal ID Requirements – New Investment Account form.

Federal ID Requirements – New Investment Account * form must be mailed

This form, required under the federal Proceeds of Crime (Money Laundering) and Terrorist Financing Act, obtains information regarding the existence of new corporate or other entity clients (e.g., corporations, trusts, partnerships, foundations, etc.) and identifies each individual who is authorized to give instructions on the account.

Federal ID Requirements – Attestation * form must be mailed

Attestation is one of the methods used to verify the identity of an authorized signatory. This form must be completed and signed by a professional accountant performing accounting service for the corporate entity.

Federal ID Requirements – Confirmation of Deposit Account * form must be mailed

Confirmation of Deposit Account is one of the methods used to verify the identity of an authorized signatory. This option may be used in the place of the Cleared Cheque method. This form must be completed and signed by a representative of the authorized signatory’s financial institution.

Existing Investors — General


Account Banking Information * form must be mailed

To be used to update banking information and must be mailed with an original void cheque (non-counter) attached.

Investor Profile & Account Objectives * form can be faxed or mailed

This form records the corporation’s account objective and investment profile. It is a requirement by securities regulators to have certain related information on file and confirmed annually.

Audit Request * form can be faxed or mailed

To be used to request confirmations, by way of Investment Account Statements, for a unitholder’s auditor in order to confirm their investments in the SHIF with Phillips Hager & North Investment Funds Ltd.

Delegation of Authority Form (previously Limited Authority Form) * form must be mailed

This document permits the Board of Directors to appoint a person who is not a signing officer of a corporation to have limited authority over the account(s) of the corporation to have the authority to carry out the range of actions on behalf of the corporation.  There are various levels of authority that can be given.

Resolution of Directors – Existing Unitholder * form must be mailed
Federal ID Requirements – Replacement Authorized Signatory for an existing account * form must be mailed

These forms are to be used to update officers of the corporation to be given authority for the Social Housing Investment Funds accounts. Please complete both documents whenever there is  a change in signing officers.
Please complete one Fed ID form for each signing officer.

Telephone and/or Internet Access Form *form must be mailed

This form is to request transactions by telephone and/or to view the corporation’s account details.

Change of Address * form can be faxed or mailed

This form is to be used to change the listed corporate address on your account.

Existing Investors — Transactions


Purchase Form * form can be faxed or mailed

This form is to be used to make subsequent deposits to your corporation’s existing Social Housing Investment Funds accounts. It asks which fund the corporation would like to invest in and how much to invest.

Redemptions * form can be faxed or mailed

This form is to be used to withdraw investment funds from your corporation’s existing Social Housing Investment Funds accounts. This form asks which fund the corporation would like to withdraw from and how the corporation would like to receive payment.

Switch Form * form can be faxed or mailed

This form is to be used to transfer funds within your corporation’s existing Social Housing Investment Funds accounts.

Corporate Account Rebalance * form can be faxed or mailed

This form is to be used to rebalance investment funds within your corporation’s existing accounts.

Enable Automatic Transactions * form can be faxed or mailed

This form is to be used to pre-authorize purchases or switches between investment funds in your existing Social Housing Investment Funds accounts.
